TMJ Disorder

Temporomandibular joint disorder encompasses conditions affecting the jaw joint, its articular disc, and the surrounding masticatory muscles. The pathology ranges from muscular tension and disc displacement through to degenerative joint changes, producing symptoms that extend beyond the jaw into the neck, ears, and cranium.

How TMJ Disorder Disrupts Far More Than the Jaw

The temporomandibular joint is one of the most complex joints in the human body, capable of hinging, translating, and rotating simultaneously during everyday functions such as speaking, chewing, and yawning. When this system is disrupted by disc displacement, muscle overload, or degenerative change, the effects radiate widely: patients present not only with jaw pain and clicking but with tinnitus, ear fullness, temple headaches, neck stiffness, and disrupted sleep. These seemingly unconnected symptoms frequently lead to years of misdiagnosis before a dental specialist identifies the TMJ as the source.

Vellum Select's Istanbul specialists provide a structured TMJ assessment combining imaging, occlusal analysis, and muscle examination to identify the specific pathology driving each patient's symptoms, then deliver targeted splint therapy, physiotherapy protocols, or surgical intervention where indicated, at 65 per cent below typical Western clinic fees.

Symptoms of TMJ Disorder

The hallmark symptoms of TMJ disorder are jaw pain or tenderness localised to the joint and the muscles of mastication, a clicking or popping sound on opening or closing the mouth, and restricted or deviated mandibular opening. Patients frequently describe difficulty chewing firm foods, a feeling that the bite has changed, and locking of the jaw in either the open or closed position. Referred symptoms include deep ear pain without infection, tinnitus, dizziness, and occipital or temporal headaches that patients often attribute to migraine. Neck and shoulder tension is common where masticatory muscle splinting transfers load to adjacent musculature.

Diagnostic Pathways

Clinical assessment measures maximum mouth opening, lateral and protrusive excursion, and documents any deviation or deflection of the mandible during opening. Joint sounds are characterised by timing and quality. Muscle palpation identifies tenderness in the masseter, temporalis, medial pterygoid, and lateral pterygoid. CBCT imaging assesses condylar morphology, the position of the condyle within the glenoid fossa, and any erosive or degenerative joint changes. MRI is the gold standard for visualising the articular disc position and its relationship to the condyle in both open and closed mouth positions.

Advanced Treatment Options at Vellum Select

Vellum Select's Istanbul TMJ specialists match treatment intensity to the confirmed diagnosis, beginning with conservative reversible measures before considering irreversible interventions.

TMJ Treatment

A hard acrylic stabilisation splint worn nightly decompresses the joint, redistributes occlusal forces, and allows muscles to return to a physiological resting length. Arthrocentesis lavage under local anaesthetic removes inflammatory mediators from the joint space in persistent cases. Surgical disc repositioning or joint replacement is reserved for end-stage degenerative disease unresponsive to conservative management. Learn more about TMJ Treatment in Turkey.
